SYSTEM VAN DIVISION

Base Pay: Loaded or Empty Solos: up to $.41 cpm
Home Time: is divided into two different division we offer both Weekly and BiWeekly -
Weekly - 3 out of 4 weekends
Biweekly - 12 days out / 2 days off;
No Force NYC, No Hazmat, No Canadian Freight
Quarterly Bonuses: $.01/mile Safety/Fuel,
$.01/mile On-Time Service, $.01/mile Productivity
Stop Off Pay:$15.00 per stop excluding pickup and final delivery stops
Layover Pay: $96.00 after 24 hours
Detention Pay: $3.00 per 15 minute increments after 3 hours. $84.00 maximum.
Short Haul Pay: 0 to 100 miles $.25 additional pay per mile. 101-200 $.07 cpm.
201-300 $.03 cpm. $35.00 minimum.
Unload Pay: $75.00 Full Trailer, $37.50 Half Trailer, $25.00 Pallet Pull Off
NYC/Long Island Pay: $50.00

VACATION PAY:
- 0-6 Months: 1 week
- 1 year: 1 week
- 3 years: 2 weeks
- 8 years: 3 weeks

***Hourly Pay***

HAZMAT is not required, but Tanker endorsement is required for Water.

Who can be covered on the insurance plans?

Legal spouse - If your spouse works (regardless of the amount of hours) and coverage is offered by the employer (regardless of the type or price of the coverage) they cannot be on the Medical/Prescription plan

Children & Stepchildren under 19
*Full-time students can be covered up to age 25 for the Medical & Vision plans and up to age 23 for the Dental plans - with proof of full-time enrollment in a high school, trade school or college

*Dependents ages 19-26 can be covered under the medical/prescription plan if they do not have coverage available through an employer - with Dependent Employment Verification Form completed

**Notification of changes must be submitted in writing within 30 days from the event (i.e. marriage, birth, loss of coverage, etc.). **

Your benefits (Life/Accidental Death & Dismemberment Insurance, Short Term Disability, Medical/Prescription, Dental & Vision) will become effective the first of the month following your 90th day of employment.

You are eligible to enroll in the 401(k) plan on the first of the month following your 6 months of employment.

The company will match $0.25 per $1.00, up to 3% of your gross earnings.

Prescriptions:
Employee pays 30% for generic and brand-name prescriptions and 100% for non-formulary prescriptions. There is a $10 minimum/$100 maximum. Mail-In service is available for maintenance prescriptions; forms are available through Highmark or by calling HR.
Dental Plan: United Concordia

Weekly Premiums:
Single -$2.71
Family - $8.00

Vision Plan: Davis Vision
Weekly Premiums:

Company-Paid Insurance Benefits
**Please note: We do not offer additional coverage.**
Life Insurance Policy:
$15,000 - Employee
$2,000 - Spouse
$1,000 - Dependents under the age of 19

Accidental Death & Dismemberment:
$30,000 - Employee
$4,000 - Spouse
$2,000 - Dependents under the age of 19

Short Term Disability:
Could pay the employee up to $250 per week (pre-tax) for up to 26 weeks. This is in the event of a sickness, surgery, injury (non-work related) or maternity leave extending past the unpaid 8-day waiting period. You must notify the Benefits Coordinator to request paperwork and start a claim.
